New Student Enrollment

 

Aeries Online Enrollment allows you to quickly start the process of enrolling your student(s) at a Lodi Unified School District school.  Student information such as emergency contacts, medical and language preference will be collected. Upon completion, the form is immediately available to the school that corresponds to your student's grade and your home address. The school will contact you with instructions for completing the enrollment process. You can enroll multiple students within a single online enrollment account. The system will copy information from the first student to the rest and allow you to edit it as needed. You must have an Aeries account to begin the process.

Things to know...

We are delighted that you will be joining us. To expedite your enrollment, please be sure to provide the information requested below.

• Proof of residence is required** - you must live within the Tokay High School attendance area or submit an Intra-District Agreement request and wait for approval; proof of residence is required in either case (ex; City of Lodi or PG&E)
 
• Legal documentation of birth—birth certificate or passport/visa

• Unofficial transcript from previous school

• Withdrawal grades – for late or mid-term enrollee
 
• Most recent IEP if Special Ed student
 
• Proof of up-to-date Immunizations -PLEASE REMEMBER: California state law requires that prior to being allowed to attend classes ALL students entering a California high school should have completed the following:
          o Tdap Pertussis—booster minimum of one dose on or after the 7th birthday (new state requirement)
          o DTP—4 doses, 3 doses OK if one was given on or after 2nd birthday
          o Hepatitis B—series of 3 vaccinations
          o MMR—Measles/Mumps/Rubella vaccination minimum of 2 are required given on or after 1st birthday
          o Polio—4 doses, 3 doses OK if one was given on or after 2nd birthday
          o Varicella—one dose ages 7-12 or 2 doses for ages 13-17 or doctor verification that the child has had the disease.
 
  • Parent/Guardian Valid Photo ID
• Guardianship Forms—If the student is not living with parent or legal guardian a Caregiver Affidavit must be completed in our office. Guardian must present picture ID along with proof of residence. Legal guardians must show court documentation.
