ALL DRIVERS MUST NOW BE FINGERPRINTED AND CLEARED THROUGH LODI UNIFIED SCHOOL DISTRICT PRIOR TO TRANSPORTING ANY STUDENT-ATHLETES

Process for Driving Student-Athletes

Step 1

Complete a Volunteer Fingerprint form with the help of the Athletic Department.

Step 2

Set up an appointment with Lodi Unified School District Personnel Department to get fingerprinted and get fingerprinted on the day of your appointment (Bring the Volunteer Fingerprint form with you).

Step 3

The Athletic Department receives clearance from the district that you have cleared fingerprinting.

Step 4

You fill out a Volunteer Driver Application and Authorization for Release of Driver Record and turn it into the athletic office or your son/daughter's coach.

Step 5

We turn in the Volunteer Driver Application and Authorization for Release of Driver Record into the district office and receive notification that you are cleared to transport students.

A completed packet includes:

  • Copy of valid Driver's License
  • Copy of current vehicle registration
  • Copy of insurance declarations including policy expiration

Step 6

School notifies volunteer of clearance to be a volunteer driver.
