Athletic Clearance

In accordance with Lodi Unified School District, California Interscholastic Federation and Tokay High School policies, all prospective student-athletes must receive clearance from the Athletic Department in order to participate in athletics at Tokay High School.

To get cleared for participation, follow the steps below.

Clearance Process

Step 1: Online Registration

  1. Visit
  2. Choose the state of California
  3. If you are new to Athletic Clearance, please watch the quick tutorial video. If you have registered before, skip to Step 5.
  4. REGISTER. Parents register with valid email username and password. You will be asked to type in a code to verify you are human. If this step is skipped your account will not activate.
  5. Login (Use same information if you registered in a previous year)
  6. Select NEW CLEARANCE to start the process
  7. Choose the school year in which the student plans to participate (2018-2019)
  8. Choose the school in which the student is enrolled at (Tokay)
  9. Choose the sport in which the student plans to participate (Ex: Football)
  10. Complete all required fields for student information, educational history, medical history, and signature forms
  11. Download and print the physical form to take to your own physician.
  12. When you reach the Confirmation Message, you have completed the online registration.
  13. If you would like to register for additional sports/activities you MUST check off those sports below the Confirmation Message
  14. All of the data will be electronically filed with the school’s athletic department for REVIEW.

Step 2: Print & Sign Confirmation

  1. Print and return the Confirmation Message (1 copy for each sport) to the athletics office or to your coach

Step 3: Physical

  1. Obtain a physical from your personal physician or a licensed health care provider. You can download and print the form on your Athletic Clearance account and you may also upload the completed physical on your account. You may also pick up a hard copy of the physical from the Athletic Office. 

Step 4: Receive Clearance

  1. When the student has been CLEARED for participation, an email notification will be sent to you and the student-athlete is eligible to participate in athletics.
