ALL DRIVERS MUST NOW BE FINGERPRINTED AND CLEARED THROUGH LODI UNIFIED SCHOOL DISTRICT PRIOR TO TRANSPORTING ANY STUDENT-ATHLETES

 

Process  for Driving Student-Athletes

 

1. We help you complete a Volunteer Fingerprint form.

2. We give you the completed Volunteer Fingerprint form.

3. You set up an appointment with Lodi Unified Personnel department to get fingerprinted.

4. Get fingerprinted on the day of your appointment (Bring the Volunteer Fingerprint form with you).

5. We receive clearance from the district that you have cleared fingerprinting.

6. You fill out a Volunteer Driver Application and Authorization for Release of Driver Record and turn it into the athletic office or your coach.

7. We turn the Volunteer Driver Application and Authorization for Release of Driver Record into the district office. 

8. We receive notification that you are cleared to transport students.

 

There are three types of people that may be interested in driving for a school activity.

 

1)      Current Employee  - current employees are already cleared through the fingerprinting process and may start on Step #6 above and fill out the Volunteer Driver Application.

2)      New Volunteers – People who are newly interested in becoming a volunteer and driving during school activities must first be cleared as a volunteer through Personnel.  Once cleared, they may then initiate the Transportation Department process for driving.

3)      If you think you have been previously cleared as a volunteer, contact Athletic Director Michael Holst at mholst@lodiusd.net. Once the Athletic Department verifies clearance, you may start on Step #6 and fill out the Volunteer Driver Application. If you are not cleared, you must start on Step #1.
